Výskyt v populaci je cca 1%, existuje genetická predispozice, vzniká nejč. ve středním věku. Projevuje se zánětem několika kloubů, obv. symetrickým, s výrazným postižením drobných kloubů ruky a dalších menších periferních kloubů horní i dolní končetiny, postižen může být jakýkoliv kloub vč. kloubů páteře, zejm. krční. Je ztluštěna synoviální membrána, v níž jsou přítomny zánětlivé buňky produkující četné enzymy a cytokiny. Zánět má ráz progresivní s tendencí k destrukci kloubních struktur s nebezpečím pozdějšího vzniku deformit (např. ulnární deviace) a znehybnění (ankylózy). Projevuje se bolestivostí, ranní ztuhlostí, mohou být podkožní revmatoidní uzlíky. Bývají rovněž celkové příznaky (nechutenství, deprese, postižení jiných orgánů aj.), takže onemocnění má multisystémový ráz. Onemocnění někdy začíná v období klimakteria, ale může vzniknout kdykoliv. Laboratorně bývají známky zánětu, anemie, obv. je pozitivní revmatoidní faktor (tzv. séropozitivní forma), v pozdějších fázích jsou typické změny rentgenologické (eroze, osteoporóza v okolí postižených kloubů). V léčbě se používají nesteroidní antirevmatika, v těžších případech kortikoidy či imunosupresiva, DMARD, důležitá je lázeňská léčba a rehabilitace, někdy se provádí chirurgický zákrok.
